It was obvious that Clive and Vernon mutually poisoned each other. What is implied to me is that George may have hired the Dutch doctor and nurse to finish them off with injections. I felt this from George's sense of triumph over Molly's former lovers (Clive, Vernon, Julian Garmony) at the very end of the novel.
