Most woman with PKD that have normal kidney function can have a safe pregnancy. When you have below normal function, your blood pressure tends to be higher and the stress of pregnancy can make it worse. If the girlfriend is close to needing a transplant, she'd be a high risk pregnancy and there could be a chance of it worsening her remaining kidney function.
This really seems like a big risk to take. If your friend wants to have a baby with this girl, they should make an appointment with her nephrologist (kidney specialist) to find out the risks involved. Otherwise he should make sure he handles his own birth control.
