I would go into the system prefs and set the alarm sound volume to what you want it to be. Then do a soft reset.

Now go into DAtebk5 and in the alarm prefs, tap the Sounds & buttons popup. (It says Tap to Select).
When that comes up, for your appt. sound, choose something different and then back to what you want it to be. You should hear the alarm play.

Now test your normal alarms.

I have had this issue on my T3 with datebk5, and I don't use any of the sounds management features of QLaunch.

Perhaps the following information will help you track down what happened. When you perform QLaunch's toggle sounds or set auto silence, it will grab your current sound setting, and then restore that later when it is turned back on, or if your turn it back on. If your sound was already off when you executed the setauto silence, or the toggle alarm sound, then it will restore volume to OFF.
