This is way out of my expertise, but here are my thoughts.

First, could he be in some type of digestive pain? You specifically mentioned hitting himself in the stomach. Maybe he's developed some type of food intolerance or allergy. In our area, there is a lot of mold, even in the winter. People are having winter allergies.

Next, I have talked to someone who used Risperdal (?) for self-injurious behaviors, and it has changed their family's quality of life.

I'm not pro-med in general unless it is a last resort, but once they reach a certain age and if they are injuring themselves and others, I'd think about it more seriously. If nothing else, you could combine it with behavioral mod. in hopes of getting him off the meds.
