You could save space by putting some of the individually-listed items in as part of the author's note -- the dedication, for example, and the feedback instructions. You could even put the disclaimer there, as I've seen many authors do. I tend to put the summary at the very end, right before the author's note. I think squishing the original publication credit and the wordcount into the same line looks odd and is a bit confusing, though. I'd list them separately.
