Is there any chance do you think that her mate may have pecked her ear? It certainly sounds like he may have been responsible for her feather loss. I was thinking after I'd posted that it may have been unlikely that Nutmeg boinked due to her splayed legs. Another thing I remembered was a Quail of mine, Widget. One day I noticed a weird scabby looking thing on her ear protruding outwards, I caught her & examined it, when I touched it with my finger it moved slightly, so I gently eased it off with my finger & it actually popped out of her ear!! On closer inspection it was actually a bit of ground corn that had somehow got lodged in her ear. Her ear looked very strange afterwards, very open where the corn had been, but a couple of days later it seemed totally back to normal. Maybe Nutmeg has somehow gotten a foreign body in her ear and it's caused a bit of infection, or maybe the 'scab' itself is the foreign body like in Widgets case? It does sound like it may be somehow infected, especially with the balance problems.
