If that is Desmond down-the-hatch, I'm worried that they'll be stretching the believability envelope just a little too far. At this point, I'm fine waiting for some explanation that fits the "he's there but then he's here" situation into the existing mythology.

But structurally, introducing the character (obviously with importance to Jack's backstory) in the very same episode that he "reappears" kind of sapped the suspense. If he had been introduced in the first Jack-Sara flashback from last season and then showed up, it would have been more satisfying.

And does anybody actually look at someone they recognize and say, "You!"??
