If it were me, I would go right ahead and buy him. Of course, have a vet check him out before you do anything, but if you click with him and he has good conformation and movement, I would buy him. The foal rating is a notoriusly unreliable predictor of what premie the horse will recieve as an adult. Many 1st premie babies have gone on to be normal, 3rd premie adults, and quite a few Approved Stallions were 3rds when they were babies.
Also, I don't know the circumstances, but it is possible he was registered without being inspected, or was lame or otherwise unwell the day of keur. Or he could have been an ugly duckling. ;o)

In my mind, if you love a horse, the premie should not stop you from buying him/her. After all, the no premie horses need homes too, and from your description, he sounds like he won't have any problem getting into the Studbook as a three year old.
