There are situations where an officer must inform their CO in a timely manner. One such situation is pregnancy - at least while an active duty Marine, an officer has to inform their CO of pregnancy within two weeks of diagnosis. I'm not sure how it is in other areas of the service, but I imagine it's probably similar in other areas and situations. I looked up the policy myself when I was writing a fic about Mac being pregnant.
