Subject: Same here. Unless otherwise instructed, I go with the phonetic pronunciation. BTW, sorry I haven't responded to your questions. I hadn't anticipated being without my book this long (two weeks in Hawaii was planned, but not this further time without my copy of the book) and I can't remember the specifics that well!
