So, she and Nicholas are in Famagusta. She's dying and there is this exchange:

He raised his head slowly. The hollow eyes on the pillow were filling with tears. He would never have said that to anyone living. He had said it because she was dying; and she knew it. The greates balm he could bring, brought on a knife-point.

What does this mean??? The greatest balm he could bring? that he had really loved Marian? that he hadn't, so Katelina could think he loved her? and the knife point is that they never had been able to?

Earlier, Nicholas got upset because he asked Diniz "What do you think you've been watching these past weeks?"
Diniz said "Compassion?"
And N walked away because he was angry. Angry that he couldn't hide that he didn't really love K? or because D was giving N more credit than N thinks he deserves because what he feels is guilt and grief. He has a bitter thought about that- D thinking him capable of more.

Help! This is the most poignant part of the series- to me- and I just can't quite get all the subleties.
