Make sure you have wheel chairs at both ends of your flights. Pack some snacks, they get oddly hungry and the most inopportune times. Plan "rests" in-between activities were you can return to your hotel and given them a few hours of a break. Have medicine on hand in case they get a cold. I've had wonderful trips with my dad, but need to go at his pace.
