Im a nurse living in a latin american country. Rectal temp checks are, for the most part, still the norm here, at least in the rural area where i work. The government has a "health for children" initiative where twice a year public school students 10 and under are driven by bus to our clinic to be examined. it takes the staff (2 pediatricians and 6 nurses, rotating in 2 shifts) a full week to check the appox. 400 kids that flood our regional clinic.

Checking their temperature is the first step in the check up. All children under 7 get their temp taken rectally. Between 7 and 10 is the nurse's call depending on the child's maturity, clinical record and disposition. The fact is, however, that rectal temp checks are much more expedient and efficient than oral temp checks (you wouldnt believe how many kids spit the therm out of their mouths just for the fun of it, or move it around their mouths making the temp inaccurate and we dont have the fancy ear temp readers). Hence, on "health for children" week, with limited time and a very long line of antsy children waiting, rectal is the way to go.

Ours is a small clinic, so its not like we can afford much privacy either. we use two side by side rooms (one for girls and one for boys, although it inevitably mixes up sometimes) and take 5 at a time to each one. While one of the nurses preps the children in one room the other 2 handle the temp checks on the adjacent room, then switch with the single nurse once the checks are done. Girls are usually easy going, although some whine about the whole experience. Boys, hwever, can get a bit feisty sometimes, especially older ones if they get stuck with a girl in the same room.

On many occasions kids that visit the clinic outside the "health for children" week are pleaseantly surprised to know their temp will be checked orally. parents sometimes ask why the difference in treatment. The fact is that with more time and less patients the staff can afford to treat the kid better, not just as 1 in 400 that need a temp check stat!!

Thank you for your post Andrea,it's good to know that the rectal method is still used in some places.Do you use digital thermometers or the old fashioned glass/mercury ones?I was a very impatient boy so I often wound up taking the thermometer in my butt even as I got older.It was just what needed to be done back then and even though I was often very embarrassed and angry about it then,I have since forgiven the women at that office for what they did to me.
