In childhood and adolescence I got injections bending over the lap of a p/a when I went to the doctor and rolling over in bed when he paid house calls. Although terrified, I must had hid my feelings because I was usually praised for taking my shots bravely. It is therefore unlikely that I ever clenched my buttocks and I certainly never screamed or cried.

If you turn your toes inward, it is much more difficult to clench your buttocks and an injection there will be less painful. It is easier to have a patient do this if he is standing and bent over and keeps his weight on his feet. You can inform a lying patient to do this as well.

Whether a child will do so though is something of a coin flip. On the other hand a child ling on his stomach is going to be able move as easily particularly if a hand is placed firmly on the back.
