I also have known nurses who have been reinstated after being revoked, but a word of warning... it will be a challenging journey and requires work and dealing with the mandates of the BON. All states have specific requirements for reinstatement and these are on the BON website, under laws/ regs/ policies.

I have heard of some who needed to take their NCLEX again, some need refresher courses, and most need CEU's.

In addition, you will need to show proof of recovery, with documents from treatment facility, counselors letters, and sponsors letters. They may require additional treatment... they will require a 5 year contract.

I know that FL BON does have a stipulation for nurses who don't want to return to clinical settings, but there is no specific licensing type for this.

You also to need to be aware of the OIG exclusion list, if you were revoked in 2001, the FL BON was required to inform the HHS department, and as a result, they place people on this list. This list prevents people from working in any capacity with employers who receive medicare or medicaid reimbursement.
