it really boils down to judgement of the official, and them understanding HOW the coach works a game...........example, if they see the guy flipping out all the time, although obnoxious, they might just ignore it until it crosses the line (vulgar swearing or making it a personal attack on them)........while others, who rarely complain, get a real long leash to vent as needed.........the officials try to be as fair and understanding as possible. As for the "out of box" scenario.............widely abused, and hardly called (unless your Calipari, LOL), much like 3 seconds or the hand check foul........seems the focus on certain types of calls change from year to year, depending on what the governing body had dictated at the beginning of the season.

Let's face it though, and I say this tongue in cheek of course................If any ref had one additional eye, they would be labeled a "cyclops"!!!!!!!!!! LOL
