I'm not sure where you fell asleep but I will run through a shortened account.

It started out with some dude exercising in what looked like an underground bunker--he was, BTW.

There were a number of flashbacks and in the main one, Jack was working in a hospital ER when a woman was brought in from a car accident. She was paralyzed and he did surgery to try and repair the damage. He told her he would fix her. After the surgery he went for a run in the stadium--trying to go up and down all the steps. There was another runner with some sort of Aussie accent. When Jack twisted his ankle and sat down, the other runner came over, introduced himself, said he had dropped out of med school or some such thing, and told Jack he wanted to run around the world.

Back to the hospital, Jack was sitting in the room waiting for the woman to wake up to tell her the bad news that he couldn't fix her. She asked him why she could wiggle her toes and guess what--she appeared to have been healed.

Back to the island, Jack wanted to wait until morning to enter the hatch but Terry O'Quinn didn't want to wait. He and the dark haired chick went down the hatch. (For some reason their names are eluding me--watched way too many new shows last week) Jack followed down into this huge room filled with equipment, computers, flashing lights, etc. Looked like a dome of some sort. Anyhow as he proceeded to look he found O'Quinn standing in a hall. Jack had his gun out but was told by a voice to drop it. It turns out that the face behind the gun was the runner dude from earlier.

I guess that's it in a nutshell.
