I think that's just a damaged/ripped mostly missing stripe.

Yes, so that would be presumably from the 1989 game, establishing the helmet design for that year.

I have a theory it's from an earlier game in 1990.

Their coach died in the offseason of that year so putting on a black stripe would make sense. Similar to what the Eagles did in 1992.

I think this is correct - I had misread earlier that the game program cover was for a game vs Eastern Illinois, but it's actually for vs Illinois State, last game of the year in 1990. Also the black stripe is visible in photographs in the 1991 WIU yearbook, so it's presumably not a damage situation.
