I have not done this, as our Sailer 29 does not have transom-hung rudder, but I think that I would remove the fitting, make good any damage to grp with expoxy filler or cloth and polyester resin if necessary, check that the backing pad on the inside of the transom is sound and adequate, then bolt the fitting (not screws) using sikaflex 291.

It might be worth looking at the top fitting at the same time.
