I think the objective of the procedure should influence the choice of position for the receiver.
If one is interested in effectiveness of the enema, the position that allows the greatest induction should be used. The best positions for this purpose involve allowing unfettered expansion of the tummy, and as close to vertical trunk of the receiver as possible, to get gravity involved with assisting the inflow. Also, slower infusion is better. Many variations are presented in other replies here.
A common position which can provide a lot of these benefits is the 'knee chest' position, knees spread apart to shoulder width (better if the knees are raised a bit so the receiver's trunk slant get closer to vertical). The knees being spread allows space for the tummy to expand.
If the objective is for play, then position should be chosen to maximize exposure or embarrassment, whatever. The 'diaper changing position' which Mum used on me (for her convenience, I believe) is considered by some to maximize exposure, possibly embarrassment. It Definitely provides ready access to most points of interest ( LoL ). The position is the receiver laying on his/her back. It is reasonably effective from the point of view of the enema, as well, as the tummy is relatively free to expand.
One of the more common positions (usually easiest for 'solo') is the Sims position, receiver laying on his/her left side, right leg bent up towards the chest. This is a reasonable position enema-wise, not as good as knee chest; perhaps a bit better than laying on the back.
They are all good. I have used them all. Usually my last in a series is taken knee chest; but this is awkward as I am self administering.
