My first impression was that this was a staged photograph. You know what? This image should have been photo-shopped to show, say an 18-19 year old female patient or a somewhat older woman.

Perhaps it was staged; maybe not. The nurse major looks like she is probably a mom herself & almost ANYthing adults can do to distract these little people can be helpful.
I was in a military clinic one morning getting an injection in my arm. At the next table was a little boy (about 5-6) & his mom. I was looking away & the little guy called to warn me that the technician was about to jab me. I told the boy I didn't want to know when I got stuck. That seemed to surprise him. I told him it didn't hurt as much if I wasn't watching when the shot was given. We discussed that for long enough that the tech was able to get the boy's injection ready & give it (the tech had asked me to keep the boy distracted if possible) while the boy & I continued the discussion. The mom thanked us both saying it was the first time her son had taken a shot without a major battle.
I think the photo simply shows an experienced nurse engaged in good patient relation technique with a child.
