Sort of looks like the young lady will be getting her temp taken rectally. Unless I'm not seeing the thermometer. It appears the caregiver is putting some lubricant (vaseline) on the girl's anus so she can easily insert the thermometer.

TRUTH be known, there is no thermomiter visible. She may be preparing her for an enema. Note the equipment on the wall in the room.

It's not a vintage photo. The older Vaseline jars were rounded and this one is more modern!
