Definitely the exposure and the lack of control over the situation. First, just being at the doctors means you are at the mercy of someone who you don't know, a doctor or nurse. Then you have to take your clothes off, which is embarrassing. But the point when a doctor or nurse examines my backside, spreading my cheeks, looking at my anus, that is the most embarrassing. The first time I remember it happening, I was about 12, and the doctor held my cheeks open for what seemed like an hour while he examined my hole, though he never touched it. Fast forward a couple decades, and I was at the doctors with a small hemmoroid (a bump) on my anus, so during three exams, I had to lie on my side with my pants around my ankles while three different doctors (two of them had a nurse with them) examined me. In one case, the nurse just stood there with a clipboard (not sure why she was there except to have a peek at someone's ass perhaps?). In the other case, the doctor used my situation to explain the exam to her and even had her put lubricant on his finger. In each case, I got a finger in my rectum (the have to check for internal bumps too). All incredibly embarrassing. Of course, when my girlfriend and I play doctor, I love when she examines my bottom.
