 It is called flat because a cross section of the stem would look like a rectangle with rounded corners. It is much wider than high. Whan you rotate that stem in the rectum, it feels heavenly, and you definitely can feel it. When the etched markings slide over the nerve endings of the anus, you feel that, too. I have a few of these, and of the B-D equivalent called the Flatype Model 150. Those were the reasons why my fetish is based on great stimulation as well as very tender bonding.
