in the 1960th and early 70th these types of rectal glas thermometers were also used in Germany in the pediatry. They looked slightly different, there was not a real pear shaped bulb but more a slight rounded cone shaped bulb on these thermometers, a thin shaft and the thicker scale part like in your pics. The bulb part was inserted till past the sphincter in the rectum, the thin shaft laid within the sphincter and the thicker part was outside. Reason was most likely to keep the thermometer in place and also for preventing perforation of the rectum when the thermometer was inserted much deeper.
