In my experience, position is not so much a factor for effectiveness as it is in comfort depending on the situation. That applies to a Fleet type enema or a regular enema with a bulb or bag.
If I use a Fleet type I have to go in a few minutes regardless of position.
If I am giving myself an enema with my bag, my position depends on the purpose, but most often I am on my back because it is comfortable and gives me access to any other part of my body I choose.

I still prefer the knee-chest position because of the memories associated with the enema.
