The reason your kids squirm, is because you are administering an enema to the child while they are prone face down over your lap. Not only is this physical uncomfortable for the child, it is the worst possible way to administer an enema. The reason why this is a poor position is the child's abdomen will be compressed as the fluid fills thier small intestine and works it way to the large intestine. This also could possible cause even more of an impaction since the fluid will start to inflate the intestinal walls since it will have no where to go as you inject more fluid into your child's rectum. The safest and most efficient way to administer an enema is to place the child on their side with their legs bent at the knees and pulled up slightly to the child's abdomen. This is called the sims position. This position allows the large and small intestine's to be uncompressed and gives a relatively clear passage for the fluid to pass through. If you must use the over lap position on your child. Place the child on their back and raise their legs just high enough to allow you a unobstructed access to the child's anus. Also always use a water based lubricant such as the brand KY. Vaseline is not water based and can cause an interior rectal rash which will cause extreme pain for the child when they have a bowel movement. Also Vaseline or other petroleum based products are impervious to water and therefore do not evacuate from your child's rectum when they expel the enema fluid. Finally, never give an enema to a crying child and never never spank your child before, during or after you have administered the enema. When a child cries they release a chemical into their body called prolactin. This hormone can cause constipation, when released in a large quantity. What that means is your administering an enema to your child to assist them in having a bowel movement, will not be helpful if your child is crying and they can become even more impacted. The rule is: an upset child's illness can not be treated until the child is treated well.
