The only time I can envision both the doctor and the patient completely naked would be if you lived in a nudist campground and the doctor came there.
So of course only one person at a time would be the patient and have to bare some or all.
One variation we had was to play that the patient was in a coma when brought into the ER and the nurse, usually my sister or my cousin Patricia, had to remove all the clothing and provide the appropriate assistance, which included a bath while in the bed. I especially liked that when it was my turn to be the medic.
I think my cousin liked the scene where she and my sister would help the unconscious patient sit at the edge of the bed and I'd have to pee into some old jar we'd snatch from the kitchen.
Evntually after my uncle came home from a hospitalization
and brought one of the plastic urinals they'd try to hold my penis in the opening of that container while I was still lying on my back. That didn't always work because when Patty touched me I'd start to get a erection which made it point straight up and not into the container. Between them giggling as it rose up and the toching sooner or later I'd not be able to pretend I was mortally wounded and have to sit or stand up.
But I think she still liked that senario.
