I saw a 15 year old in my clinic today that came in for a pelvic exam. As I performed the bimanual portion of the exam, she almost came off the table. I made a slide and discovered she had a vaginal infection, a yeast infection, and I sent off cultures for 2 other sexually transmitted diseases. I treated the 2 infections I knew she had with oral antibiotics, but fearing she wouldn't return, I ordered a shot of rocephin as well. I very rarely see my patients receive the injections that I order, but there was a lot to talk about and she didn't speak english, so I had to use an interpreter. The interpreter explained the medications and what they were for and she seemed to understand. Then the interpreter told her she was going to get a shot. She rolled her eyes and tried to refuse. The interpreter told her how important it was and that she could keep hurting or get the shot.
My nurse, Carmen came in and told her "Lie down for me, honey - this one goes in your bottom!" Of course she didn't understand and Carmen patted the table. The girl sat on the exam table looking petrified and took off her sweater. Carmen said, "No - in your bottom..." very slowly. The interpreter said it in spanish and she shook her head. The interpreter told her to lie down in a stern voice and she slowly unzipped her jeans.
She told the interpreter that she was very embarassed for so many people to look at her bottom, but gingerly obeyed and laid down on her tummy. She slid down her jeans - she wasn't wearing any panties - and exposed her brown bottom.
Carmen said, "I'm sorry honey.." and jabbed her with the needle. As she injected the girl took a sharp breath in and began to moan. She whimpered and moaned for the full injection and when she sat up there were tears streaming down her face. Carmen apologized again and gave her a tissue. I finished my chart and left the room, but I saw her and the interpreter walking down the hall a couple of minutes later and the girl was still crying and rubbing her right cheek. She rubbed it until they left the building. I'm glad I gave it to her because the culture came back positive.
