Has a very short "to the point" waiver form.
or at http://www.sunless.com on the Airbrush Tanning Messageboard, under "Best Posts For newcomers", are some detailed examples of both a client waiver form and a Body Chart to note scars etc. You can also download an editable word version.
They are good forms to have on hand. But there is alot of controversy on weither they will actually help you. And many lawyers say that they will not protect your business or personal assets if a lawsuit is involved. So you really need to discuss it with your attorny.
