We had an identical experience with crystals and quality of the dress. I feel strongly that we should be able to call out publicly dressmakers who do this (my experience is only one, and we have had a number of new dresses made and shipped from o/s). I think it is the same shoddy dressmaker, but what can we do, other than what another poster correctly advised, which is to order without crystals and try and go local? Also teachers have about 95% input in our school.
