Subject: urinating in the house

We have a 3 year old jack russell (female) in which we got her from the spca. She was having accidents in the house before and we took her in to the vet and they found a hernia. They operated and it was fine. She started having the same problem again and this time they found crystals in her urine. They put her on medicine and everything was fine. She is now having the same problem but the urine sample came back negative for everything. We dont know what we can do next. We tried a crate and she goes in there (thats were she was kept as a puppy and her owners never let her out) (not us), if we confine her to an area she goes there, we also have tried the diapers and she goes in them also. We are getting very frustrated and dont know what the next step should be. Any suggestions? Thank you
