 I guess, since the doors to exam rooms open onto a common hallway where patients and medical professionals travel, you come into an exam room and, usually open, there is a curtain which can be drawn around the exam table area so that, with a patient undressed on the exam table, should the door open (when the doctor comes into the room, for example) should others be right behind him or her, no one would be able to look at the patient.
