I don't ever remember a female official. In high school our trainer for a couple of years was a woman, if she needed to examine us or tape us we'd be in a jock strap at a minimum.
In middle school most guys wore something under their singlets, some guys would double up on underwear to keep things snug if a boner popped up which was less common than you'd think but definitely happened. I mean we were teenage boys and in certain holds and positions it would start to get hard. Typically the ref would stall a bit and let you make it look like you got the wind knocked out of you so you could let the boner go down before starting the match again.
In high school I stopped wearing anything under my singlet mostly because it was more comfortable as underwear always finds it way into every nook and cranny and I wasn't embarrassed about what god had given me. I'd say it was about split down the middle if guys went commando or not.
Like I said before there are a lot of positions and fast grabbing and erections happen, but the focus is on winning so they tended not to last long, everyone once and a while there was a guy on the other team that was just wrestling to feel you up, but that usually lead to a quick pin. At the tournament level those guys are pretty much weeded out.
