I think id be looking for a differant D.R. !!! at this age this has gone on too long, and it isnt healthy.
could the problem have gone from fear of the toilet, to chronic constipation?????holding it will cause it to harden, and make it very uncomfortable passing. you said you tried fiber, but have you tried a stool softener, like colace???? something to actually soften it??? or mineral oil??? and is he drinking enough water?????? is he on any medicines that could cause constipation????
if it were my kid, i think the immediate problem would be making sure he had a good cleaning out first, then find a way to address the problem of holding it in,and looking for a 2nd opinion from a D.R. that will listen.
