I, too, have a 6yr old puff with renal issues. First, it sounds like you have a supportive vet and that's half the battle. Second, while many pets push the KD or other prescription foods, they are too low in protein & high in phosphorus to really get the results that you need. I HIGHLY recommend you join the K9KidneyDiet@yahoogroups.com chat list. They will require that you post you boy's bloodwork results in order to better help you manage his CRF. My puff was diagnosed in February with astronomical BUN, Creatinine and Phosphorus levels. By starting him on Azodyl, Gastricalm, daily subq fluids (100ml) and a home cooked high quality protein/low phosphorus diet, we have his numbers back down to normal (BUN/CR still a touch elevated but excellent compared to where he was 3 months ago). The K9KidneyDiet group was a Godsend for me, there is SOO much conflicting information out there and they helped me make sense of it all. They have TONS of information in their files section, including diet calculators, recipes, supplements information (Bandit takes several different supplements daily in addition to everything else we're doing).
You can definitely prolong your baby's life by carefully monitoring his diet, daily subq fluids, etc. COntact your vet immediately and ask about starting your boy on Azodyl, it is a probiotic which helps reduce blood uremeia (BUN/Creatinine); how is your dog's appetite? Remember that CRF dogs can be picky eaters and when their phosphorus levels are elevated, it kills their appetite, can make them nauseous (that's where the Gastri-Calm really helps - another probiotic) just makes them feel crappy in general.
