Subject: Borrelia, nerve blocke or?

I was hoping that maybe somebody could help me on this complicated problem.
I have a horse that momentarily becomes extremely lame on his left forelimp *( we are talking about grade 5). This has been going on for 2½ months now. When he gets an "attack" he will not put weight on the leg for approximately 5 min and will only put the front part of the hoof on the ground. He does not seem to be in pain while having the attack as he concentrate on other things than his leg (but I can't be sure). He has no swelling og heat in any part of his leg. He's shows little lameness after an attack which has been contiunosly on and off. The lameness is visible when being asked to trot clockwise at the end of a long line (Longering it's called in my country), then he is lame on this left limp.
His carpus, fetlock and hoof has all been x-rayed and nothing abnormal has been found. My vet wanted to apply a local anaesthetic into each joint (starting at the hoof) hoping that we would find the troublesome joint ( I want to add that he has been examined for tendor lesiosns but negative results). On the day for the exam he was not lame enough to do the examintation. My vet suggested that I should start riding him again and try to provoke a detectable lameness (this was last week).
I had a physiotherapist to look at him a couple of days ago. She informed me that my gelding has muscle atrophy and reduced neurologic reflex on his left side of the neck (the triangular area just obove the shoulder), and muscle atrophy over the shoulder. The right side has little atrophy as well but nothing much. She also informed me that he had a lot of muscle pain/filtration in his neck (jsut behind his ears). The physiotherapy had a tremendous positive effect on him.
After this long intro My question is has anybody experienced anything similar?
I have been looking on the internet about disease and found two diseases that discribes some of his symptoms. The first is Borrelia/Lyme's disease (he was bitten by a tick this spring) and radial nerve damage, but I was also thinking that maybe the muscle tensions in his neck might be squeezing his nerves?
Do you have suggestions?
