I need help please with Hershey wrapper

Hello,
I wondered if anyone can help with something for my church. One lady at the church loves to make a welcome gift for the new folks and guests (of course her name is Karen..sweet and helpful).
Now she is baking and would like something a bit easier on her.

She would like a wrapper to go on the large size 5oz Hershey bars. I can't..so I thought I would ask here.
She would like something like:

WELCOME
Pathways Christian Fellowship
South Wales, New York

also maybe a small image of Praying Hands? or
a bible verse? to jazz it up a little

This would be greatly appreciated. Any changes you need or want to make are fine.
