Dear Karen,
HIBM looks very much like Inclusion Body Myositis on muscle biopsy, with the exception that there is not very much inflammatory changes with HIBM. The distinction CAN be made clinically and by history of disease course, pattern of distribution and weakness, age of onset, etc...
