I read somewhere that the actor playing tank's role (sorry, don't know his name) messed up with the W brothers big time and they wrote him out.
I don't think his death is explained in detail in any of all matrix stories (movies, game, animatrix), so we must assume he died of his injuries or so...

hope not to disappoint you with this, I liked tank very much in the first movie, I also think he's a lot cooler than link...
