i have 3 rotts 1 male 5 yrs. 2 females 7 yrs. & 2 yrs. the youngest is the one daughter of the other two. I have experienced a couple of things similar to this one was doggie zits (under the chin) so nasty they were like ours only huge! Like tons of little ones so close that they made a huge one (the size a quarter at least). They did seem to be painful to them (only father & daughter had it) the first time we used antibiotics (I have a close relationship with everyone at the vet), we all thought that they were zits (he's realistic with me)their anatomy isn't that different than ours (maybe some extra stuff in different places but still the same)What is good for us is generally good for them. After the antibiotic (big blue pills) I would put something on it as well, clean area with peroxide then put neosporin on it, that will help. Or there's a product called Schreiner's (www.schreiners.com)you can put that on it also it helps with the irritation so she'll let it alone. Now my other thought, missing hair, thick skin. Is it sticky too with a bit of an odor? If yes, that almost sounds like a sort of a staff infection (it can and will kill her if let go) it will start small then progressively get bigger, this doesn't just affect that area it will spread throughout her whole body from the inside out. I deal with this in the summer when it's really hot out. You can't prevent it just watch for it closely. What we do is go to the vet for antibiotics (big blue pill) he also gives a shot then we start with the ritual. 2 times a day rinse area with peroxide then use Malaseb shampoo (from vet, I keep on hand all the time now) wash area really good to remove dead skin & puss, rinse, wash area again really good and let soak for 10 minutes, rinse, dry then use ChlorHex 2x(also from vet I keep on hand too)spray on and leave. This needs to be done 2 times a day. The areas are extremely painful for them and if let can kill the dog. I hope this helps, as I said we go through this in the hot months (the last 2-3 yrs.) all you can do is watch closely for a sticky spot.
