For one thing, a black hole of very small size radiates a LOT of energy, and would cook your brain like a bratwurst in a steel smelter. A small enough black hole would actually explode with roughly the power of a thermonuclear bomb, according to calculations.

A larger black hole--the size of a grain of rice, say--would be so massive it would fall through your body like a bullet and head for the center of the Earth. A black hole roughly the size of a small grape would be as massive as the Earth itself.

Black holes are a physical phenomenon, not a mental or emotional one. They can't swallow thoughts and emotions, but they can and would do massive tissue damage if they popped up inside your head. If you suspect your brain has sustained massive tissue damage, go to a hospital at once. ;)
