The midline being off usually refers to the condition where the space between your top two front teeth doesn't line up with the space between your bottom two. (I say "space" in the sense of where the two teeth meet, not in the sense of a big gap or anything.) Midline problems can also occur when the space between your two front teeth isn't on the center line of your face (i.e. right under the middle of the bridge of your nose). The second case can occur when an ortho overcorrects for the first case - there's some debate as to whether it's better to get the midlines of the two jaws to match, or to get the midline of the upper jaw to match the midline of your face.

Like most things related to your teeth, nobody will notice this as much as you do. My boyfriend's midline is off both ways after jaw surgery - his top two front teeth aren't in line with the center of his face /or/ with his bottom two front teeth. I'm sure I'm the only one who notices, and I still think he has a great smile. :)
