It sounds like the upper arch is just too narrow. Another RPE might help, surgery might be an option. It is really hard to tell without seeing photos, x-rays and the like.

The other option i to close the ext spaces and stay in a bit of cross bite, again hard to tell you what to do without actually seeing you. Wish I could be of more help.
