I have the fall 2009 newport p.s. (pleasure scene) book and in the middle of book it saids you can send in your UPC's and receive 3 different items I want $25 goods discover card but there is no form to fill out or mailing address. I have done this before and there has always been a form and mailing address. How can I get this offer it is almost to late deadline is 12/31/09. Please give instructions for this offer.
