Do you get these flashes before you've worked out what the plot is going to be?

Yes - usually the story comes from a scene, a very brief sense of movement - sometimes a setpiece - sometimes the scene is the beginning, sometimes its the end, sometimes it's the middle. Sometimes its almost nothing more than a sense of movement and emotional color, if that makes any sense at all, sometimes just a still image - and then it clarifies and takes shape. Then, sort of like stringing beads, more scenes will come to me and I slowly get them ordered in my head.

I have outlines and sense of where things are going..... but the journey can and does take me far afield. Take the medieval saga, for instance, part of the reason i stopped where I did is that I realized that I didn't like the pattern of beads I'd strung out in my head. I still know what a few of the key anchor ones are, but the rest I've pulled out and I'm working on imagianing new ones and getting a pattern togehter in my head - still working out from the few key scenes - but changing it up to reflect my new ideas.

The dialogue emerges as much to connect the beads as anything else, and it feels right to me when it feels in character, and heads in the direction I want it to go. Which can take a lot of deleting. *g*

so, um yeah. very slow process....
