I'm not sure it's mentioned in the books directly. I know Leto II mentions the name used in the Bene Gesserit breeding records, but I'm not sure Mohiam is directly stated.

I think this is one of the tales told in his short stories and notes used by Brian and Kevin to write the preludes.

When they talk about DUNE 7 in the FAQs page on the official site, they say before they wrote the final story of the DUNE universe, they had to do the preludes to give readers important background information.
