In "The Adventure of the Solitary Cyclist" we learn that Holmes has had "..a few unavoidable failures." These were never chronicled. Most of these cases I believe were never referred to by name, and Dr. Watson alluded that Holmes was called in much to late and/or there was way to little evidence or facts were available.

Please Note: I do not consider cases such as "The Five Orange Pips" failures.
