Usually for championship races such as District, State or National Championships, a fairly flat course is used so that overall speed is the key and is fairly comparable from race to race. Some 40km time trials are even held on courses and in places that are very conducive to fast time, ie. Moriarity, New Mexico. Flat and at altitude.
The sub one hour 40km is about the equivilent of the sub 4 minute mile in running or sub 30 minute 10km, or a sub 18 minute 1500m swim.
