I tend to think Spike meant 'always' as in 'always has the potential', not as in 'something will always happen'.

One could argue that Tara, despite being an experienced and cautious practitioner of magic, was storing up a weight of unpaid consequences for herself which eventually resulted in her seemingly meaningless and random death.

The problem I have with this is that I think witches would have known about this problem, and it would have been made clear in the show at some point. And if it was so certain then there would be no witches left.

I tend to think that magic is a dangerous and tricky business, but that if you *fully* know what you are doing you can avoid the pitfalls. And other times you might get lucky.

But fallout from amateur magic making was often a very handy plot device.
